Attention Redington Shores Residents:
Registration for COVID-19 vaccinations for anyone 65 and older in Pinellas County re-opens at 10 a.m. Saturday, Jan. 23, following a new shipment of 8,000 doses from the State of Florida. Appointments start early next week and are expected to fill quickly. More information can be found on the Pinellas County website: https://covid19.pinellascounty.org/vaccines/
In alignment with the State of Florida, patients will now be asked to show proof of Florida residency to receive a vaccine. Examples include: a valid Florida Drivers License, state issued ID, deed, or utility bill with the patient’s name and address. Part-time residents will be asked to provide part-time rental agreement. Please understand demand is still very high with a limited supply.
Here is a checklist to make it clear what information is required to create an account and sign up for an appointment.
COVID-19 Vaccination Pre-Registration Check List:
To create your account at www.patientportalfl.com you will need the following information:
1 – Enter your full name, date of birth, phone number, email, home address (only persons with a Florida address will be vaccinated), gender, race, and ethnicity. You will also need to be ready to provide some information about your medical history at the time of booking the appointment.
2 – For the insurance related fields, you may submit it or click “Decline to Answer”. Insurance is not required to receive the COVID-19 vaccine and you will not be billed for this service.
3 – A notepad or somewhere you can write your username and password to use later. To schedule your appointment when slots become available: 1 – Create or access your account at www.patientportalfl.com or sign in using your username and password 2 – Complete the COVID-19 Vaccine Liability Release Waiver 3 – Complete the Past Medical History, Family Medical History, and Relevant Medical History questionnaires
4 – Enter your Zip Code and/or Desired Appointment Date
5 – Select the site closest to you
6 – Select your desired Appointment Date/Time slot for your First Vaccination
7 – Select your desired Appointment Date/Time slot for your Required 2nd Booster Vaccination
8 – Once you have selected your appointments you will receive a confirmation screen.
***Please print, or screen shot, the QR code for your first vaccination appointment and present the QR code at check-in during your appointment.
